Social Worker Care Manager

What is the position?

We are looking for a Social Work Care Manager to help care for Oscars’  members.

First and foremost, this job is about being an amazing social worker. This includes being excited about creative ways to engage members and provide ongoing guidance and support.

The social worker will work with Oscar’s Concierge Team and will collaborate closely with nurse partners to help support Oscar members.  

Deliverables: What you have to achieve

  • Care Planning
    • In coordination with nursing develop an outcome-based plan of care, based on the member’s input, with anticipated targeted dates for completion
    • Identify and addresses biopsychosocial needs of the members and family
    • Communicate and document each member’s plan of care in an organized, goal oriented manner
    • Maintain a comprehensive list of available wellness and other community programs available to members and facilitate referral of member to resources
    • Assist members with end of life planning
  • Member Engagement
    • Be available (during work hours) to take incoming phone calls from assigned members
    • Outreach members to identify and help solve for barriers that may be affecting their care.
    • Have knowledge of community resources and how to leverage them to assist members
  • Care Tracking
    • Continuously coordinates, monitors, tracks and evaluates all care and services rendered to ensure that quality care is being delivered in the most appropriate setting
  • Care Transitions
    • Manages transition of care, ensuring that the Plan of Care moves with the member and updates/modifies the care plan as the member’s health status changes
    • Responsible for the coordination of post-discharge clinic appointments, medication reconciliation, PCP and specialist visits
    • Facilitate transition of care to medicare or medicaid when appropriate
  • Resource Connection
    • Maintain a comprehensive list of available wellness and other community programs available to members and facilitate referral of member to resources
    • Have knowledge of community resources and how to leverage them to assist members.
    • Facilitate transition of care to Medicare or Medicaid when appropriate
    • Assist with applications for disability and resources as appropriate
    • Share resource expertise with team members and serve as social work subject matter expert for team

Winning factors: What leads to success

Must-have qualifications and expertise include:

  • Masters in Social Work (MSW) required from a graduate school approved by the Council on Social Work Education
  • LMSW/LSW Required
  • At least 2 years of medical social work experience
  • Familiar with medical case management and healthcare
