Abortion Care Program Coordinator (Navigator)

POSITION TITLE: Abortion Care Program Coordinator/Navigator
STATUS: Full Time, Exempt – 37.5 hours per week
STARTING SALARY RANGE: $46,000
SCHEDULE: Monday thru Friday

Position Description
Under supervision from Patient Care Operations Leadership, the Abortion Care Navigation Coordinator (referred to throughout as Navigator) will participate in all aspects of planning for and supporting the abortion patient’s needs to access care on both an individual basis, and at a systems level. The Navigator helps guide the patient through the abortion care system and works to overcome obstacles faced by the patient receiving the care and services they require, while working to improve the systems and processes that shape access at the clinical and ecosystem levels.  In their role as a Navigator, the employee will be the “point person” for PPMI abortion navigation. The Navigator will be familiar with our in-house abortion services as well external services and resources. This staff person will fully embody a commitment to race equity, evidenced in all interactions with PPMI colleagues and stakeholders.
 
Essential Functions
·       Assists in the rollout of abortion navigation services across all affiliate health centers
·       Facilitates referrals to outside providers. Develops and maintains a list of external providers in the country providing care complementary to that provided at PPMI
·       Organizing schedules and managing appointments for patients to ensure they receive services in a timely manner and at a location most convenient for them based on the patient’s clinical care site preferences
·       Facilitating communication between the patient, support person(s) and/or organization(s), and healthcare providers to ensure patient satisfaction and quality of care.
·       Informing the patient on financial aspects of care and linking the patient to financial resources as needed
·       Working with the patient to identify logistical needs to access care and linking the patient to appropriate resources as needed
·       Working with the patient to identify other support services as needed, and providing appropriate linkages
·       Facilitating pre and post-abortion visits and ensuring continuity of care and linkage to community resources
·       Collecting, tracking, and reporting required program data
·       Oversight of AB funding sources; including creating process for, oversight of, and completion of invoicing, reconciliation, and data reporting
·       Identifying and addressing existing gaps and opportunities within existing processes, including screening processes, to ensure highest-need patients are being identified, routed, and connected to navigational support
·       Working with external stakeholders and organizations to build and strengthen relationships, systems, and processes that support access to abortion care
·       Working with external vendors to facilitate access to key services, including but not limited to, transportation and lodging
·       Working closely with affiliate and contact center staff, health center managers, and others to create efficiencies and improve workflows related to abortion navigation
·       Maintains up-to-date materials related to abortion services, including patient information sheets and resources
·       Assists in outreach and engagement with internal and external stakeholders. Helps build relationships with other abortion navigation programs across the country
·       Attends abortion care conferences and events deemed applicable to the job duties of the Navigator.
·       Participates actively in the PPFA Abortion Navigator meetings
·       Participates in, and leads as appropriate, training for stakeholders in abortion care navigation, including patient care staff and in other departments as required

Required

Preferred
·       Experience in community based medical/health or social services settings; social work, counseling, nursing, public health or other health education
·       Knowledge of abortion laws, and resources
