Mobile Outreach Program Manager

TLDR

Lead the development and implementation of a mobile clinic program supporting homelessness outreach, care coordination, and health navigation at Covenant Community Care.

Covenant Community Care, a faith based charitable non-profit Community Health Center serving the people of Metro Detroit, has created a new opportunity for a Mobile Outreach Program Manager to oversee the development and implementation of a remote/mobile medical program that provides service to people experiencing homelessness in Metro Detroit.

This position will develop and oversee the implementation of a mobile clinic program at Covenant Community Care that primarily serves people experiencing homelessness including: Care coordination for homeless patients and securing assistance with accessing care at Covenant clinics;  patient recruitment and outreach strategies to increase and sustain patient volumel; health navigation services such as: health insurance enrollment assistance, choosing a Primary Care Provider, and scheduling/making appointments.

Wil also Represent Covenant Community Care by providing education and participating in community events as well maintain appropriate data and metrics related to number of patients assisted and services rendered.

The successful candidate will posess a Bachelor's degree, prefrably in a health-related field and prior program management experience. Direct experience in working with underserved and special needs population in a healthcare environment is highly preferred.  Must have reliable transportation and a valid driver's license.
