Patient Navigator (Remote)

The Opportunity

The Patient Navigator works with providers and care team to provide quality care service to members enrolled in our TIMAP (Transgender, Intersex, Medical Advocacy Program).  The Navigator is responsible to support the member throughout their care plan and is integral in identifying gaps in preventative care services, coordinating referrals with key specialty providers, encouraging follow through on physician recommendations.  As the patient’s primary contact, an effective Patient Navigator will build and maintain relationships providing empathetic and emotional support for TIMAP members. 

 


Responsibilities

  • Educate eligible members on the TIMAP program and resources within and external to Teladoc Health.
  • Ensure TIMAP members are welcomed into the program in a supportive manner.
  • Capture each members’ expectations, goals, needs from the program including personal preferences.
  • Support members, physicians and care team to prioritize case management and demonstrate an appropriate sense of urgency throughout the case.
  • Manage, organize and update relevant data using case management software.
  • Ensure accurate, concise and thorough case documentation in the case management system in a timely manner.
  • Provide regular case updates to the member and escalate case information as needed to the physician and clinical team.
  • Prepare physician and provider referrals.
  • Provide exceptional service to internal and external customers including but not limited to clinicians and members.
  • Effective monitoring of incoming email and faxes.
  • Ensure the highest quality and timeliness for all outgoing reports and communications.
  • Schedule member appointments with Teladoc clinical staff.
  • Conform with and abide by all regulations, policies, work procedures and instructions.
  • Participate in meetings and special projects as needed.

Candidate Profile

  • Experience, knowledge of and sensitivity to gender dysphoria, LGTBQ+ and transgender needs and support
  • Proficient computer skills, specifically with Microsoft software
  • Strong time management skills
  • Ability to effectively manage multiple, competing priorities
  • Excellent verbal and written communication skills
  • Ability to work independently and as part of a high performing team
  • Comfortable recommending changes to improve member experience and team performance
  • LPN, Medical Assistant or other healthcare background helpful
  • Ability to thrive in fast-paced work environment in a growing company
  • 3+ years experience working in a customer focused environment preferable in healthcare or hospital setting
  • Medical vocabulary a PLUS
  • Bachelor’s degree in Healthcare or Business Administration preferred
