Medical Assistant - Indianapolis, IN

Job Description:

The Medical Assistant serves as a critical operational and clinical support partner within Cityblock Health’s integrated care model. Working closely with assigned providers and interdisciplinary care teams, the Medical Assistant is responsible for supporting pre-visit planning, visit readiness, care coordination workflows, Athena inbox management, and post-visit follow-up activities to ensure high-quality, coordinated, and member-centered care delivery.

This role plays an essential part in supporting provider efficiency, continuity of care, quality gap identification, operational consistency, and member engagement across daily workflows. The Medical Assistant functions within a highly collaborative environment and is expected to demonstrate strong operational ownership, communication, accountability, and adaptability within evolving care delivery models.

Core Responsibilities

Pre Visit Planning and Visit Readiness

  • Conduct comprehensive pre and post visit chart preparation prior to scheduled appointments.

  • Verify insurance eligibility, registration accuracy, demographic information, and appointment readiness prior to visits.

  • Complete confirmation calls and member outreach within established workflow expectations.

  • Engage members to schedule and confirm annual visits, discharge follow up appointments, chronic disease management visits, and preventative care services.

  • Support identification and closure of HEDIS and quality gaps, preventative screenings, BOI capture opportunities, and required questionnaires.

  • Ensure relevant labs, imaging, referrals, prescriptions, and external documentation are available, reviewed, or escalated prior to visits.

  • Identify scheduling discrepancies, operational barriers, and visit readiness concerns proactively.

  • Ensure quality and patient outcome opportunities are elevated for provider review and intervention.

Dyad Huddle Participation and Provider Partnership

  • Participate actively in daily dyad huddles, recurring provider meetings, and workflow discussions.

  • Review provider schedules, operational priorities, member overlap, coverage needs, and visit readiness requirements.

  • Prepare and maintain dyad huddle tracking tools, workflow documentation, and operational follow up items.

  • Support provider coverage coordination and escalation management as needed.

  • Maintain professionalism, preparedness, and active engagement during huddle participation.

  • Collaborate with providers, operations, nursing, behavioral health, pharmacy, and interdisciplinary teams to support coordinated care delivery.

Athena Inbox, EMR Management, and Documentation

  • Manage assigned Athena inbox tasks within established SLAs and workflow expectations.

  • Prioritize, route, and resolve operational and clinical workflows accurately and timely.

  • Review, track, and support follow up on lab results, referrals, prescriptions, imaging, provider orders, and outstanding care coordination activities through EMR management.

  • Maintain accurate, timely, and up to date patient records within the electronic health record system.

  • Document member outreach, appointment updates, care coordination activities, and workflow completion accurately.

  • Escalate high priority, unresolved, or clinically sensitive tasks appropriately.

Clinical Support and Care Coordination

  • Assist healthcare providers with examinations, diagnostic testing, clinical procedures, and specialized procedures as authorized within scope of practice.

  • Administer medications and vaccines under the direction of a licensed healthcare provider.

  • Coordinate patient care activities including scheduling appointments, obtaining referrals, managing follow up activities, and supporting continuity of care.

  • Support follow up related to durable medical equipment (DME), labs, imaging, prior authorizations, prescriptions, and provider orders.

  • Track and communicate care coordination updates impacting member care and operational workflows.

  • Proactively escalate unresolved care coordination barriers and member needs.

Administrative, Inventory, and Operational Support

  • Answer phone calls and respond to member inquiries professionally and compassionately.

  • Greet members and visitors entering physical hub locations and support a welcoming member experience.

  • Assist nursing and hub operations teams in maintaining medical inventory and ordering supplies and materials as needed.

  • Monitor and manage inventory of medical supplies, medications, and clinical materials to support operational readiness.

  • Participate in workflow improvement initiatives, operational discussions, and process standardization efforts across markets.

  • Maintain respectful, transparent, and solution oriented communication with peers and interdisciplinary teams.

  • Support a collaborative, accountable, and member centered team culture.

EXPERIENCE

  • 1 year of experience working in a clinical operations setting

We take into account an individual’s qualifications, skillset, and experience in determining final salary. This role is eligible for health insurance, life insurance, retirement benefits, participation in the company’s equity program, paid time off, including vacation and sick leave. The actual offer will be at the company’s sole discretion and determined by relevant business considerations, including the final candidate’s qualifications, years of experience, skillset, and geographic location. The expected salary range for this position is: 

$20.19- $25.96
